Palliative Care Kit

reachAbility is in the process of creating a Palliative Care Kit. Its purpose will be to provide support and guidelines for patients and their families in such a time of crisis.

We would like to establish a step-by-step kit with parameters on what to do, when to do it, and who to call when one needs further information and help. We also intend to create a section on Rights & Responsibilities, and on Wills & Bequeaths, in plain language.

To create this kit, we have requested the full support and cooperation of the Palliative Care Unit at the Victoria General Hospital in Halifax, to make sure that it will be continually distributed and hence used.
